Pinnacle Community Services aims to provide a range of high quality services and support for older people, people with a disability and their families and carers to help them to live with dignity and independence in their own homes in the community.

Pinnacle Community Services offer a range of Community Home Support Programme (CHSP) services and will work with you to help you create customized support and services that suit your needs. Pinnacle Community Services can also work with you to help facilitate and manage your Home Care Package.

Our services can be accessed by having an assessment to ensure you meet the eligibility criteria. People 65 years and over or 50 years and over for Aboriginal and Torres Strait Islander can contact the My Aged Care team on 1800 200 422 (or see My Aged Care for more details) to arrange an assessment.

People 64 years and under or 49 years and under for Aboriginal and Torres Strait Islander should refer to our Disability Supports section to view what services we offer and how they can access these services.


COMMUNITY TRANSPORT

Temora Community Transport offers a local bus service Monday to Friday 9am-5pm. It offers a weekly bus service to Wagga every Thursday.

The centre offers a volunteer driven car service for clients to access appointments in other towns, for clients that have no alternate transportation available. Bookings are essential and fees are charged according to distances travelled.


FOOD SERVICES

Freshly cooked hot meals are available in the Temora township delivered 6 days per week with an extra meal for Sunday. These includes main meal and sweets with options. Menus change regularly andclients are able to oder their meal preferences on a weekly basis.

Frozen Meals are also available in Temora plus throughout the Shire, including Ariah Park, Springdale, Yanco and other villages.


HOME MAINTENANCE & MODIFICATION

This service specialises in maintenance, modifications and adjustments required to Clients homes. The purpose of this service is to make the home safer and more accessible to the occupant. This is includes (but is not limited to) the installation of railings, changing light bulbs, completing lawn maintenance.

Clients will be asked to pay for the costs of materials and a contribution towards labour.


SOCIAL SUPPORT - Individual

This volunteer run service can assist clients with shopping, reading letters/papers, accessing the community or offer a friendly visit to the home for a chat and a cup of tea.


SOCIAL SUPPORT - Group

This service offers group outings to events around Temora and the surrounding regions. It also offers a monthly meet and eat at Temora Deli. 


RESPITE CARE

Respite is designed to be responsive to the needs of the carer. Unless otherwise specified, respite services will be provided in the home of the client. Respite services are provided on a one to one individual basis. Respite is generally provided on a planned basis. 


CENTRE BASED DAY CARE - DEMENTIA SPECIFIC

This service offers a day program for people who are suffering from a mild to moderate dementia.

The service also provides valuable respite for the carers. Respite is offered each Thursday from 9.30am to 3:00pm from the Temora Day Care House.
A contribution towards the service is asked to cover the cost of morning tea and lunch.


DOMESTIC ASSISTANCE

Provides assistance with Domestic Tasks such as vacuuming, mopping, changing sheets etc. This service also includes a laundry service that provides washing, drying and folding of laundry for clients who are unable to complete this task themselves. This service includes the pick up and delivery of laundry.


PERSONAL CARE

Provides assistance with Self Care including showering assistance with dressing and support with medication.
